Family Check-In(sm)
Family Check-In(sm) is a service that checks on your Loved One(s) that may be at Home, in Assisted Living, or Long Term Care. You are the first to realize that when you cannot visit a loved one as often as you would like, you cannot get involved in the direct monitoring process. At best, you are relying upon someone else's assessment of the care your Loved One is receiving.
Our Service provides an independent assessment that you can monitor and control. Our staff, all with a minimum of a Bachelor's Degree or RN Degree, has the acuity to make an unbiased assessment on the overall environment and care being received.
A Report is sent to you after each visit. You decide on the construction of the check-list. A check list will contain items that concern you or recommendations that you may ask us to make. A report might contain:
*cognitive status *personal hygiene
*functional environment *diet compliance
*social interaction *hydration
